By enrolling, the enrolling party accepts full financial responsibility for tuition and fees. The registration fee is non-refundable. Tuition refunds upon voluntary withdrawal are governed by the schedule below: written notice received prior to the first day of instruction entitles the enrolling party to a refund of tuition less non-refundable registration fee; withdrawal within the first two weeks entitles the enrolling party to a pro rata refund based on instruction delivered less administrative charges; after two weeks no tuition refund is available. Late payments are subject to a stated late fee and collection costs; the Academy may suspend access to instruction for delinquent accounts.

Enrollment Agreement: The enrolling party hereby enrolls the student in the Academy program identified above. The enrolling party acknowledges receipt of and agrees to comply with Academy policies, including but not limited to attendance requirements, academic standards, conduct rules, and payment obligations. Failure to comply may result in suspension or dismissal. The Academy reserves the right to amend schedules and policies with prior written notice to enrolled families.

Authorization for Emergency Medical Treatment: In the event of an emergency and the undersigned cannot be reached, the undersigned authorizes Academy representatives to secure necessary emergency medical treatment for the student. The undersigned agrees to be financially responsible for such treatment.

Photo and Media Release: The Academy may photograph or record students during instruction and school activities for educational, promotional, or archival use. Check below to indicate consent.

Records and Privacy: The Academy maintains student records necessary for academic and administrative purposes. Requests for student records must be made in writing.
