Policies, Code of Conduct & Liability

Enrollment Acceptance: Enrollment is not final until this form is signed and any required deposit is received. The program reserves the right to cancel classes for insufficient enrollment. If a class is canceled by the program, tuition paid will be refunded in full or applied to another session at the payer's election.

Payment & Refund Policy: Tuition is due upon registration unless a payment plan is agreed in writing. Deposits are non-refundable. Refunds prior to the first class are subject to a processing fee. After instruction begins, refunds are prorated at the program's discretion; no refunds will be issued after the second class session except for documented medical withdrawal. Returned payments, collection fees, and late fees are the responsibility of the payer.

Code of Conduct: Students are expected to follow instructor directions and behave respectfully. The program may dismiss a student for behavior that endangers others or significantly disrupts instruction; dismissal does not automatically entitle the payer to a full refund.

Materials and Personal Property: Some materials may be provided; students may be required to supply additional materials. The program is not responsible for loss or damage to personal property brought to class.

Photo & Likeness Release: I grant the program the right to photograph or record the student during class and to use images for promotional, educational, or archival purposes unless I indicate otherwise by checking the box below.

Liability Waiver and Assumption of Risk: Participation in art activities involves inherent risks including, but not limited to, cuts, chemical exposure, dust inhalation, and allergic reactions.
