The undersigned authorizes Deaf Action Center to provide program services and to make reasonable, nonexperimental, emergency medical decisions in the event that the undersigned cannot be reached. The undersigned also provides permission for staff to communicate with emergency personnel and treating medical professionals as necessary.

Release of Information: The undersigned authorizes Deaf Action Center to obtain and share relevant information concerning the student with schools, medical providers, and other service providers when necessary for coordination of services. This authorization is limited to information directly necessary for program participation and care and will remain in effect until revoked in writing by the undersigned.

Media Release: I consent do not consent to have the student photographed, videotaped, or recorded for program promotion, educational documentation, or internal training. Consent may be revoked in writing at any time.

Attendance, Withdrawal and Refunds: Enrollment constitutes a binding agreement for the selected program and the tuition specified. Withdrawal requests must be submitted in writing. Fees for services rendered prior to withdrawal are non-refundable. Refund eligibility, if any, is determined according to Deaf Action Center's published refund policy and program-specific terms. The Center reserves the right to suspend or terminate services for failure to comply with program rules or nonpayment.

Behavioral Standards: The student is expected to adhere to Deaf Action Center policies regarding conduct and safety. Repeated or severe behavior that endangers self or others may result in immediate dismissal from the program without refund.

Financial Responsibility: The undersigned agrees to be financially responsible for tuition and fees incurred and authorizes Deaf Action Center to pursue collection of unpaid balances, including reasonable collection costs and attorney fees where allowed by law.
