Enrollment and Tuition: Enrollment is effective upon acceptance by the swim school and payment of required fees. Guardian is responsible for timely payment of tuition and fees. Late payments may incur administrative fees and suspension of lesson privileges until accounts are current.

Assumption of Risk and Liability Release: Swimming and aquatic activities involve inherent risks, including but not limited to slips, falls, collisions, and drowning. In consideration of the swim school's acceptance of this enrollment, the undersigned, on behalf of the student and the undersigned's heirs, assigns, and personal representatives, hereby releases, waives, discharges, and covenants not to sue the swim school, its owners, directors, instructors, employees and volunteers (collectively "Released Parties") from any and all liability for any loss, damage, injury, or death, and agrees to indemnify and hold harmless the Released Parties from any claims arising out of participation in swim school activities, except for gross negligence or willful misconduct by the Released Parties.

Medical Authorization: If the student requires emergency medical treatment and the guardian cannot be reached, the undersigned authorizes the swim school staff to secure emergency medical care and transport to an appropriate medical facility. The undersigned agrees to be financially responsible for any such treatment.

Health Representation: The undersigned certifies that the student is physically able to participate in swim lessons and has provided complete and accurate medical information above. The undersigned agrees to notify the swim school promptly of any change in medical condition or communicable disease.

Photo/Video Release: From time to time, the swim school may take photographs or videos of classes and events. The undersigned grants the swim school the right to use such photographs and recordings for promotional, educational, or archival purposes without compensation.

Attendance, Make-up and Refund Policy: Lessons missed by the student may be made up only in accordance with the school's published make-up policy and subject to availability. Refunds are provided only as described in the school’s cancellation and refund policy; fees paid are otherwise non-refundable for sessions already started.

Code of Conduct: Students are expected to follow all safety instructions, remain respectful to staff and other participants, and refrain from unsafe or disruptive behavior. Repeated violations may result in suspension or dismissal without refund.
