Waiver, Release and Acknowledgments

Assumption of Risk: I acknowledge that participation in the program may involve inherent risks, including but not limited to injury, illness, or property loss. I voluntarily assume full responsibility for all risks associated with participation, whether known or unknown, foreseeable or unforeseeable.

Release and Waiver: In consideration of the student’s enrollment, I, on behalf of the student and our heirs, assigns, personal representatives and next of kin, hereby release, waive, discharge and covenant not to sue the educational institution, its board members, employees, agents, volunteers, contractors and affiliates (collectively “Institution”) from any and all liability, claims, demands, causes of action or losses arising out of or related to any injury, damage or loss sustained by the student during participation in the program, except to the extent caused by the Institution’s gross negligence or intentional misconduct.

Indemnification: I agree to indemnify, defend and hold harmless the Institution from and against any and all claims, suits, liabilities, damages, losses and expenses (including reasonable attorney fees) arising out of or resulting from the student’s acts or omissions, or breach of this Waiver.

Medical Authorization: If the student becomes ill or injured while under the Institution’s supervision, I authorize the Institution to obtain emergency medical treatment as may be necessary. I consent to medical examinations, diagnostic procedures and treatment by licensed medical personnel and agree to be responsible for all costs incurred for such treatment.

Compliance with Policies: I acknowledge receipt of and agree to comply with all applicable Institution policies, rules, and safety protocols. Failure to comply may result in disciplinary action, up to and including removal from the program without refund.

Media Release: I authorize the Institution to photograph, record, and use still or moving images of the student for educational, promotional, and archival purposes, without compensation. If I do not agree, I will leave this box unchecked.

Severability: If any provision of this Waiver is held invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Waiver constitutes the entire agreement between the parties related to the subject matter herein.
