By signing this Enrollment Form the undersigned agrees to pay tuition, fees, and any applicable costs associated with the selected program. The institution reserves the right to withhold transcripts, certificates, and graduation clearance until all financial obligations are satisfied.

Refunds: The registration fee and kit fee are non-refundable after kit distribution. Deposits are refundable only if written cancellation is received more than seven (7) calendar days prior to the program start date, minus any administrative processing. Refunds for tuition after program start are based on the institution's published pro rata schedule measured from the first scheduled day of attendance; partial attendance does not constitute completion. The student must request withdrawal in writing to be eligible for a refund calculation.

Late Payments: Late or missed payments may incur late fees and may result in suspension from classes until the account is current.

Academic Standards, Attendance & Conduct

Attendance: Students are required to attend all scheduled theory and practical hours. Excessive absences, tardiness, or failure to complete required practical hours may result in probation, denial of graduation, or dismissal. Make-up work is permitted only by prior arrangement and at the discretion of the instructor.

Grading & Completion: Students must maintain minimum competency and achieve passing grades on required assessments and practical evaluations to be eligible for program completion and state licensing application. Final determination of competency rests with the program director and faculty.

Professional Conduct & Health Safety: Students must follow sanitation and safety protocols, including disinfection procedures and personal hygiene requirements. Noncompliance may result in removal from practical activities and disciplinary action.
