The student agrees to participate actively and comply with all program requirements, schedules, and directions provided by program staff. The student will conduct themselves consistent with professional and institutional standards and immediately report any hazards or incidents to program supervisors.

Health, Safety, and Emergency Information

Authorization for emergency treatment if a parent/guardian cannot be reached: I authorize program staff to obtain emergency medical treatment as reasonably necessary and agree to be responsible for related costs.

Fees, Payment and Refunds

The student (or responsible party) agrees to pay the program fee as set forth below. All stated fees are due according to the schedule below unless alternative arrangements are approved in writing by the institution.

Refunds: Except as expressly stated in an attached written agreement, refunds will be considered under the institution's published policy. Partial attendance does not waive fee obligations unless otherwise approved in writing.

Code of Conduct, Attendance and Evaluation

The student must adhere to the institution's standards of behavior, attendance, confidentiality, and professional conduct while participating in the experience. Violations may result in disciplinary action or immediate removal from the program at the institution's discretion.

Confidentiality, Records and Media

The student shall maintain confidentiality of privileged information observed during the experience and shall not disclose confidential records or personal data except as authorized. The institution may collect evaluations and records of student performance for academic and administrative purposes.

Media Release: I grant the institution the right to photograph, record, and use images or recordings of the student for educational, promotional, or archival purposes unless I decline by checking the box below.

Liability, Insurance and Indemnification

Assumption of Risk and Release: Participation in the program may involve inherent risks. To the fullest extent permitted by law, the student (and parent/guardian, if applicable) assumes all risks arising from participation and releases, holds harmless, and indemnifies the institution, its employees, agents, and affiliates from claims, liabilities, losses, or damages arising out of or related to participation, except where caused by the institution's gross negligence or willful misconduct.

Insurance: It is recommended that the student maintain health insurance coverage. The institution's insurance may be limited; the institution is not responsible for personal property loss.

Termination, Withdrawal and Amendment

Either party may terminate this Agreement for material breach with written notice. The student must provide written notice of withdrawal. The institution may terminate participation for safety or conduct reasons without refund except as required by applicable policy. Any amendment to this Agreement must be in writing and signed by both parties.

Dispute Resolution and Governing Law

The parties agree to attempt in good faith to resolve disputes through informal negotiation. If unresolved, disputes shall be resolved by binding arbitration administered in accordance with established arbitration procedures, with each party bearing its own costs unless the arbitrator orders otherwise. This Agreement is governed by the laws of the state in which the institution's principal campus is located, without regard to conflict of laws principles.
