I, the undersigned parent or legal guardian, certify that I have authority to act for the student named above and hereby grant permission for participation in the trip described above. I understand participation involves potential risks, including transportation risks, and I accept and assume those risks on behalf of the student.

Release of Liability: In consideration of the student being allowed to participate, I, on behalf of myself, my heirs, assigns and the student, waive, release, and discharge the school, its officers, employees, volunteers, and agents from any and all claims, demands, causes of action or liability arising out of or related to the student's participation, except for claims arising from the gross negligence or intentional misconduct of the school.
