Consent, Release, and Assumption of Risk

By signing below, I, the undersigned parent or guardian, certify that I am authorized to grant permission for the student named above. I acknowledge and agree that participation in the trip involves inherent risks and hazards, which may include transportation incidents, slips and falls, exposure to environmental conditions, and other risks not reasonably foreseeable. I voluntarily assume all risks associated with my child's participation.

I hereby authorize and consent to any emergency medical or surgical treatment deemed necessary by qualified medical personnel for the student while participating in the trip, if I cannot be reached. I agree to be financially responsible for the cost of such treatment and any transportation to obtain medical care.

In consideration for my child's participation, I, on behalf of myself, my child, and our heirs, release, waive and discharge the school district, its officers, employees, agents, volunteers, and chaperones from any and all liability, claims, demands, causes of action, or damages arising from or related to my child's participation in the trip, whether caused by negligence or otherwise. I further agree to indemnify and hold harmless the released parties from any claim brought by or on behalf of my child arising out of the child's participation.

This authorization, release, and indemnity is intended to be as broad and inclusive as permitted by applicable law. If any portion is held invalid, the remainder will continue in full force and effect. This agreement will be governed by the laws of the State in which the school is located.
