By signing this Agreement, Student (and Parent/Guardian, if Student is a minor) acknowledges that participation in fishing instruction and related activities involves inherent risks, including but not limited to slips, falls, water hazards, puncture wounds, hook injuries, hypothermia, exposure, and other hazards. Student voluntarily assumes all such risks and agrees, to the fullest extent permitted by law, to release, waive, and forever discharge Instructor, Instructor's employees, agents, volunteers, and affiliated organizations from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, injury, or death that may be sustained by Student, whether caused by negligence or otherwise.

Participant agrees to follow all safety instructions, wear required personal flotation devices when on or near water, and comply with all applicable rules and instructions from Instructor. Failure to comply may result in removal from the lesson without refund.

Acknowledgement of Waiver and Release: I have read and understand the Assumption of Risk, Waiver and Release above and agree to its terms.
