In consideration of allowing Student: to participate in the Student Fitness Program, I certify that the student is physically able to participate in fitness activities. I acknowledge that participation involves inherent risks including but not limited to musculoskeletal injury, heat-related illness, and other injuries that may result from strenuous activity. I voluntarily assume full responsibility for any risks, injuries, or damages arising from participation.

I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE the school district, school, program administrators, instructors, coaches, volunteers, and other representatives (collectively, the Released Parties) for any and all liability, claims, demands, or causes of action for property damage, personal injury, or wrongful death arising out of or related to participation, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

Emergency Medical Authorization

If medical treatment is required as a result of participation, and I cannot be reached in a timely manner, I authorize program staff to obtain emergency medical care and to transport the student to an appropriate medical facility. I understand that I am responsible for any costs of emergency medical care and transportation.

I grant permission for the student to be photographed or recorded and for such media to be used by the school or program without compensation, subject to applicable policies and confidentiality protections.
