Medical Information & Authorization

Medical Authorization: I authorize school personnel or designated agents to obtain emergency medical treatment for the student if necessary. I understand that all reasonable efforts will be made to contact the parent/guardian prior to treatment when feasible. I agree to be financially responsible for medical services rendered.

Consent for emergency treatment: I consent to emergency medical treatment as described above.

Eligibility, Academics & Conduct

Academic Eligibility: Student must maintain minimum academic standards established by the school and athletic program. Failure to comply with academic eligibility rules may result in suspension from participation.

Code of Conduct: The student and parent/guardian acknowledge receipt of and agree to abide by the team and school code of conduct including attendance at practices and meetings, respect for coaches, officials, teammates, and opponents, and acceptance of disciplinary measures for violations.

Conduct acknowledgment: Student and parent/guardian acknowledge and agree to comply with the code of conduct and disciplinary procedures.

Risks, Waiver, and Indemnification

Assumption of Risk: Participation in football involves inherent risks including, but not limited to, serious bodily injury, paralysis, or death. The student and parent/guardian voluntarily assume all risks associated with participation, whether known or unknown.

Release and Waiver: To the fullest extent permitted by law, the student and parent/guardian hereby release, waive, discharge and covenant not to sue the school, district, coaches, agents, volunteers and employees from any and all liability for injuries, damages, claims, demands, or causes of action arising out of participation in football activities except for willful misconduct or gross negligence.

Indemnification: The student and parent/guardian agree to indemnify and hold harmless the school, district, coaches, agents and volunteers from any claims, demands, losses or liabilities, including reasonable attorney fees, arising out of the student’s participation or breach of this contract.

Medical Testing, Concussion Protocol, and Safety

Drug and Alcohol Testing: Student agrees to comply with the school's substance-use policies, including reasonable suspicion or random testing as authorized by school policy. Positive tests or refusal may subject the student to disciplinary action, up to and including suspension from the team.

Concussion Acknowledgement: The student and parent/guardian acknowledge receipt of the school’s concussion information and agree to follow the return-to-play protocols. Any suspected concussion will be reported and the student will be withheld from participation until cleared in writing by an appropriate health care professional.

Acknowledgement: Student and parent/guardian acknowledge and agree to the concussion management procedures.

Equipment, Uniforms, and Fees

Equipment Use and Return: Student agrees to be responsible for issued equipment and uniforms. Lost, destroyed or unreturned items may be subject to replacement fees assessed by the school.

Payment Agreement: Parent/guardian agrees to pay the specified fees according to the school's payment schedule. Nonpayment may result in ineligibility for participation.

Travel and Media Release

Travel Consent: Student may travel with the team for away games and related events. Parent/guardian consents to travel under supervision of school staff and authorized chaperones.

Media Release: Student and parent/guardian grant permission for the student’s name, image or likeness to be used by the school for legitimate educational, promotional or athletic purposes unless the parent/guardian indicates otherwise below.

Opt-out of media release: I do NOT consent to the use of the student’s image or likeness.

Acknowledgments and Signatures

By signing below, the student (or the parent/guardian if the student is a minor) and the authorized school representative acknowledge that they have read, understand and agree to the terms, policies, and conditions contained in this Student Football Contract. This agreement is effective for the season identified above unless otherwise modified in writing and signed by both parties.
