In consideration of the student’s participation in the above-described group activity, the undersigned hereby acknowledges that participation involves inherent risks, including but not limited to bodily injury, illness, permanent disability, or death, and damage to or loss of property. The undersigned voluntarily assumes all risks associated with participation and agrees that the school district, institution, its trustees, employees, volunteers, agents, and representatives (collectively the Institution) shall not be liable for any injury, loss, or damage arising from participation, except to the extent caused by the gross negligence or willful misconduct of the Institution.

The undersigned releases, waives, and discharges the Institution from any and all claims, demands, actions, or causes of action, whether known or unknown, arising from participation in the activity, and agrees to indemnify, defend, and hold harmless the Institution from any claims brought by third parties arising from the undersigned’s or student’s acts or omissions.
