I agree that the participant's primary health insurance is responsible for payment of medical services and that the Program Provider and its personnel are not financially responsible for such treatment.

I voluntarily assume all risks of participation and agree to release, indemnify and hold harmless the Program Provider, its directors, employees, volunteers, agents, and facility owners from and against any claims, liabilities, losses or expenses arising from participation, except for willful misconduct. This release includes claims based on negligence, whether active or passive, on the part of the released parties.

I further agree to comply with all safety rules, instructions and policies established by the Program Provider and understand that failure to comply may result in suspension or dismissal from activities without refund. I will notify the Program Provider promptly of any changes in the student's medical status or emergency contact information.
