Acknowledgements, Assumption of Risk & Release

I, the undersigned parent or guardian (or participant if 18 years of age or older), acknowledge that participation in lacrosse involves inherent risks, including, but not limited to, serious bodily injury, concussion, permanent disability, paralysis and death resulting from collisions, falls, equipment failure, acts of other participants, and other hazards associated with vigorous athletic activity. I understand that protective equipment does not eliminate all risk of injury.

In consideration of the participant's opportunity to participate in team activities, practices and games, I hereby knowingly and voluntarily agree to assume all risks associated with participation and release, waive, discharge and covenant not to sue the School/Organization, its trustees, officers, employees, volunteers, coaches, agents and affiliates (collectively the Released Parties) from any and all liability, claims, demands, losses or damages on account of injury, illness, death, or property damage arising out of or related to participation, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I further agree to indemnify and hold harmless the Released Parties from any loss, liability, damage or cost, including reasonable attorneys' fees, arising out of any claim made by or on behalf of the participant, including claims alleging negligent acts or omissions by the Released Parties.

Medical Treatment Authorization

If the participant becomes ill or injured and the parent/guardian cannot be reached, I authorize the coach, trainer or representative of the School/Organization to obtain and approve emergency medical or surgical treatment for the participant. I understand that I am responsible for any costs incurred as a result of such treatment.

Concussion & Head Injury Acknowledgement

I acknowledge that I have received and reviewed information regarding concussions and head injuries associated with sports participation. I understand the signs and symptoms of concussion and the importance of reporting suspected concussion to team medical staff and following appropriate return-to-play protocols before resuming participation.

Transportation Consent

I consent to the participant being transported by coaches or authorized volunteers to and from team activities and understand that such transportation may be by private vehicle or other means arranged by the School/Organization. I release the Released Parties from liability for claims arising from such transportation, except for willful or wanton misconduct.

Photography & Media Release

I authorize the School/Organization to use photographs, video, or other media of the participant taken during team activities for educational, promotional, or informational purposes. I understand no compensation will be provided and that identifying information will not be used beyond routine captioning without separate consent.

Code of Conduct & Equipment Responsibility

I agree that the participant will follow all rules, policies and instructions provided by coaches and officials. The participant will use required protective equipment and acknowledge that failure to follow rules or wear proper equipment may result in suspension from participation. I accept responsibility for the proper care of any equipment issued to the participant.
