STUDENT LESSON AGREEMENT

This Student Lesson Agreement (the "Agreement") is entered into between the parties identified below for the provision of private instructional lessons under the terms and conditions set forth herein. Provider and Student/Parent agree that lessons are educational services subject to the schedules, payment requirements, cancellation rules, and policies in this Agreement.

Late Payment: Payments not received within days of the invoice due date will incur a late fee of . Provider reserves the right to suspend lessons for delinquent accounts.

CANCELLATION, RESCHEDULING, AND MAKE-UP POLICY

Student/Guardian must provide at least hours notice to cancel or reschedule a scheduled lesson. Failure to provide required notice will result in forfeiture of the lesson fee. Provider will make reasonable efforts to offer a make-up lesson in the event of Provider cancellation or in accordance with the Provider's published make-up policy.

Exceptions: Emergency cancellations and documented school closures may be eligible for make-up lessons at Provider's discretion. Make-up lessons must be scheduled within days.

MATERIALS, EQUIPMENT, AND PRACTICE

Student/Guardian agrees to obtain required books, instrument, or materials. Provider may charge for consumables or replacement materials; such charges will be disclosed in advance and are payable by Student/Guardian.

HEALTH, SAFETY, AND EMERGENCY AUTHORIZATION

In the event of a medical emergency, if Provider cannot reach the emergency contact, Provider is authorized to obtain emergency medical treatment for the student as reasonably necessary. Student/Guardian authorizes Provider and medical personnel to take action to protect the student's health.

RECORDING, PHOTOGRAPHY, AND COPYRIGHT

Provider retains copyright in original lesson materials, arrangements, and curriculum created by Provider. Student/Guardian may not reproduce or distribute Provider materials without written permission. Personal recordings by Student/Guardian are permitted only if Provider consent is given below.

Student/Guardian consents to audio/video recording of lessons by Provider for instructional, promotional, or archival purposes.    No recordings permitted by Provider

LIABILITY, INDEMNITY, AND INSURANCE

Provider will exercise reasonable care in the conduct of lessons. Student/Guardian acknowledges that participation in lessons carries inherent risks. Student/Guardian releases and holds harmless Provider, its agents and employees from claims, liabilities, or expenses arising from ordinary risks associated with lessons, except to the extent caused by Provider's gross negligence or willful misconduct.

Student/Guardian agrees to indemnify Provider for any damages arising from Student's negligent or intentional misconduct.

TERMINATION

Either party may terminate this Agreement upon written notice. If Student/Guardian terminates without cause, prepaid lessons may be refunded according to Provider's refund policy less any outstanding balances. Provider may terminate for nonpayment, breach, or disruptive conduct after providing days' notice.

ACKNOWLEDGMENT

By signing below, Student/Guardian and Provider acknowledge that they have read and understood this Agreement, accept its terms, and agree to be bound by them. This Agreement constitutes the entire agreement between the parties regarding lessons and supersedes prior discussions and representations.
