Acknowledgements, Waiver & Release

Assumption of Risk: The undersigned understands that participation in martial arts involves rigorous physical activity, contact, and inherent risks of injury, including serious bodily injury and death. The undersigned voluntarily assumes all risks associated with participation and training.

Release and Waiver: In consideration of enrollment, the undersigned releases, waives, and forever discharges the school, its instructors, employees, agents, and affiliates from any and all claims, demands, liabilities, or causes of action arising out of or related to participation, except for willful misconduct or gross negligence proven in a court of competent jurisdiction. This release includes claims for negligence, negligent supervision, and negligent instruction.

Indemnification: The undersigned agrees to indemnify and hold harmless the school and its representatives against any loss, liability, damage, cost, or expense (including reasonable attorneys' fees) arising from claims brought by third parties as a result of the participant's actions or failure to follow school rules.

Medical Authorization: In the event of an emergency, the undersigned authorizes staff to obtain medical treatment for the participant and authorizes medical providers to administer necessary emergency care. The undersigned is responsible for all costs associated with medical care.

The student agrees to abide by all school rules regarding conduct, respect for instructors and fellow students, attendance, and safety protocols. Failure to comply with policies may result in suspension or termination of enrollment without refund for serious or repeated infractions.
