Refund and Cancellation: Except as otherwise required by law, refunds will be provided only as follows: full refund if cancellation is received in writing at least fourteen (14) calendar days prior to the program start date; 50% refund if cancellation is received between seven (7) and thirteen (13) calendar days prior to the program start date; no refund for cancellations less than seven (7) calendar days before the program start date.

Participation Consent: I hereby consent to the student’s participation in the Math Adventure Program and acknowledge that participation involves intellectual challenges, teamwork activities, onsite problem-solving sessions, and travel between program venues as described above.

Assumption of Risk and Release: By checking the box below, the signer acknowledges and agrees that participation carries inherent risks, including but not limited to: physical injury during activities, psychological stress from competitive environments, and travel-related risks. The signer releases, waives, and discharges the Program Organizers, their officers, employees, volunteers, and agents from all liability, claims, demands, actions, or causes of action for any loss, damage, or injury to person or property arising out of participation, except to the extent caused by gross negligence or willful misconduct.

Medical Authorization: In the event of injury or sudden illness, if I cannot be reached, I authorize program staff to obtain and consent to emergency medical treatment for the student. I agree to be financially responsible for such treatment and to indemnify the Program for costs incurred on the student's behalf.

Photo and Video Release: Program activities may be photographed or recorded. I grant the Program a perpetual, royalty-free, worldwide license to use, reproduce, and publish photographs, video, and student work for educational and promotional purposes.
