Attendance & Conduct: I understand that regular attendance and active participation are required for program success. Repeated lateness or misconduct may result in suspension from sessions without refund. I agree to the Student Math Stars Code of Conduct.

Cancellation & Refund Policy: Tuition refunds for withdrawal requested in writing prior to the program start date will be subject to a processing fee equal to 10% of the tuition. No refunds will be issued after the program start date except for serious documented medical emergencies subject to administrative approval.

Emergency Medical Authorization: If I am not available, I authorize program staff to seek emergency medical treatment for the student and to transport the student to an appropriate medical facility. I understand I am responsible for costs of medical care.

Release and Indemnification: In consideration of participation in Student Math Stars activities, I, on behalf of myself and the student, release and hold harmless Student Math Stars, its directors, employees, volunteers, and agents from any and all liability for personal injury, property damage, or other loss arising from participation, except to the extent caused by gross negligence or willful misconduct. I agree to indemnify and defend Student Math Stars against any third-party claims arising from the student’s acts or omissions.

Educational Records & Data Use: I authorize Student Math Stars to collect and maintain educational and attendance records for program administration and assessment. I consent to the aggregation of non-identifying academic progress data for program evaluation. Personal contact information will be used to communicate program updates, billing, and emergency notices.
