Program Policies, Consents & Releases

Confidentiality: Mentors and program staff will maintain confidentiality of personal information shared in mentorship sessions except where disclosure is required by law or necessary to prevent imminent harm. The Program Provider will notify the Parent/Guardian when disclosures are made pursuant to mandatory reporting obligations.

Code of Conduct: The Student agrees to participate respectfully and comply with program rules. The Program Provider reserves the right to suspend or terminate participation for behavior that threatens safety, undermines program objectives, or violates applicable policies. No refunds or reinstatement are guaranteed upon termination for cause.

Liability Release: To the fullest extent permitted by law, the Parent/Guardian (for minors) or Student (if adult) releases the Program Provider, mentors, volunteers and affiliates from liability for incidental injury, loss, or damage arising from participation, except when caused by gross negligence or willful misconduct of the Program Provider.

Background Checks: Program mentors and other personnel may be subject to background checks. By signing this Agreement, the Parent/Guardian or Student consents to such screening when required by program policy.

Media & Data Use: The Program Provider may collect and use de-identified program data for quality improvement and reporting. Photographs or recordings of participants may be taken for program documentation and promotional materials; a separate media release consent is requested below.
