I hereby request that the named student be permitted to participate in the Event identified above and agree to the following authorizations and acknowledgements. Select all applicable:

I consent to the student's participation and certify that the student is physically able to participate in the activities described.

I authorize the student to be transported by vehicles arranged by the School/Outing Club for the Event, including privately contracted transport, and agree that seat belts or other safety restraints will be used when provided.

I grant permission for photographs, audio, or video recordings taken during the Event that include the student to be used for educational, promotional, or archival purposes by the School/Outing Club, unless I have indicated otherwise in writing to the School/Outing Club prior to the Event.

Release and Waiver: To the fullest extent permitted by law, the undersigned releases, waives, and discharges the School/Outing Club, its agents, employees, volunteers, officers, and affiliates from any and all claims, demands, or causes of action for personal injury, property damage, or wrongful death arising out of participation in the Event, whether caused by negligence or otherwise, except where arising from the School/Outing Club's willful misconduct or gross negligence.

Indemnification: The undersigned agrees to indemnify and hold harmless the School/Outing Club for any claims, losses, liabilities, damages, or expenses (including reasonable attorneys' fees) resulting from the undersigned's negligent or willful acts or omissions, or those of the student.

Emergency Medical Authorization: If emergency medical treatment is necessary and reasonable efforts to contact the Parent/Guardian listed above are unsuccessful, the undersigned authorizes the School/Outing Club and its agents to secure medical treatment for the student, including transportation to a medical facility, and agrees to be financially responsible for such treatment.

Representation: The undersigned represents that all information provided on this form is true and complete to the best of their knowledge. The undersigned understands that falsification or omission of material information may result in the student's removal from the Event and potential disciplinary measures.

Governing Law and Severability: This Agreement shall be governed by the laws of the state in which the School/Outing Club is located. If any provision is held invalid, the remaining provisions shall remain in full force and effect.
