Code of Conduct and Attendance

Participant agrees to abide by all program rules, to attend scheduled sessions, and to conduct themselves in a manner that does not endanger others or damage property. Violations may result in suspension or dismissal without refund.

Acknowledgement of Risk; Release and Indemnity

Participant and Parent/Guardian (if participant is a minor) acknowledge that participation involves inherent risks, including but not limited to physical injury, illness, or property loss. Participant voluntarily assumes all such risks and agrees that the Program Provider, its affiliates, employees, volunteers, and agents shall not be liable for injury, death, or loss arising from participation except to the extent caused by the Program Provider's gross negligence or willful misconduct.

Participant and Parent/Guardian release, waive, and discharge and agree to indemnify and hold harmless the Program Provider from all claims, demands, causes of action, suits, damages, losses, or expenses (including reasonable attorneys' fees) arising out of or related to participation, except where such claims arise from the Program Provider's gross negligence or intentional misconduct.

Medical Authorization

In the event of an emergency, the undersigned authorizes the Program Provider to seek medical treatment for the participant. The undersigned agrees to be financially responsible for any medical treatment and releases the Program Provider from liability associated with emergency medical care, except in cases of gross negligence or willful misconduct.

I grant the Program Provider the right to photograph, record, and use images or recordings of the participant for educational, promotional, and archival purposes in any medium, without further notification or compensation, unless I check the box below to withhold consent.

Privacy and Confidentiality

The Program Provider will collect and maintain participant information required for program administration. Personal information shall be used only for program-related purposes and handled in a manner consistent with applicable privacy obligations. The Participant consents to such limited use and to reasonable disclosures to emergency personnel or as required by law.

Termination; Refunds

The Program Provider reserves the right to suspend or terminate participation for failure to comply with program rules, safety concerns, or nonpayment. Refunds, if any, will be issued in accordance with the Provider's published refund policy and may be prorated for time not yet provided.

Governing Law and Dispute Resolution

This Agreement shall be governed by the laws of the state or jurisdiction in which the Program Provider operates. Any dispute arising under this Agreement shall be resolved through good faith negotiation; if negotiation fails, the parties may pursue mediation or other consensual alternative dispute resolution prior to commencing litigation, except where injunctive relief is necessary.
