I, the undersigned, acknowledge that participation in the program or event identified above involves inherent risks, including but not limited to physical injury, illness, property damage, and other risks that may result from travel, activities, equipment, or the actions of other participants. I understand these risks and voluntarily assume all such risks on behalf of the participant.

On behalf of myself, the participant, and our heirs, executors, and assigns, I hereby release, waive, discharge and covenant not to sue the school, institution, organizing entity, and their trustees, employees, volunteers, agents, and representatives (collectively Released Parties) from any and all liability, claims, demands, losses or causes of action, whether known or unknown, arising out of or related to participation in the activity, except to the extent caused by the gross negligence or willful misconduct of the Released Parties.

I further agree to indemnify and hold harmless the Released Parties from any suits, actions, damages, claims or liabilities (including attorneys' fees and costs) arising from participation in the program, whether caused by the participant, third parties, or otherwise, except to the extent arising solely from the gross negligence or willful misconduct of the Released Parties.

In the event of injury or illness, I authorize the Released Parties to obtain medical treatment for the participant and to transport the participant to a medical facility as appropriate. I understand that reasonable efforts will be made to contact the emergency contact listed above prior to medical treatment when feasible.

I grant permission for photographs, audio recordings, or video recordings taken during the event that include the participant to be used by the school or organizing entity for educational, promotional, or informational purposes without further consent or compensation unless I have expressly indicated otherwise.

If the participant is under 18 years of age, I certify that I am the parent or legal guardian and that I consent to the participant's participation and to the terms of this Waiver. This Waiver is binding upon me, the participant, and our heirs and legal representatives.

If any provision of this Waiver is found unenforceable, the remaining provisions shall remain in full force and effect. This Waiver constitutes the entire agreement between the parties with respect to the subject matter herein.
