I authorize the school and its representatives to obtain emergency medical treatment for the student named above if I cannot be reached. I understand and agree that I will be financially responsible for any medical services provided on my behalf or my child's behalf. I further authorize the release of necessary medical information to medical personnel providing treatment.

Assumption of Risk, Release and Indemnification

In consideration for the student being permitted to participate in the activity or program named above, I, on behalf of myself, my heirs, executors, administrators and assigns, acknowledge and agree that participation involves inherent risks, including bodily injury, illness, death and property damage. I knowingly and voluntarily assume all risks associated with participation, whether known or unknown.

I release, waive, discharge and covenant not to sue the school district, school, program organizers, volunteers, employees, agents and contractors (collectively the Released Parties) from all liability for any loss, claim, demand, action or cause of action whatsoever for personal injury, property damage or wrongful death, arising out of or resulting from the student's participation, except to the extent such loss or injury arises from the gross negligence or willful misconduct of a Released Party.

I further agree to indemnify, defend and hold harmless the Released Parties from and against any and all claims, demands, damages, liabilities and expenses (including reasonable attorneys' fees) resulting from the student's participation or any breach of this Release by me or the student.

Conduct, Safety Rules and Disciplinary Measures

The student agrees to follow all rules, safety instructions and directives of program staff and volunteers. The student may be removed from participation without refund for failure to comply, for conduct that endangers self or others, or for violation of school policy. Parents/guardians will be responsible for transportation home in such an event if requested by program staff.

I grant permission for the student's image, voice and/or likeness to be photographed, recorded or otherwise documented during program activities and to be used by the school or program for educational, promotional or archival purposes.
