This Student Participation University Games Agreement (the Agreement) establishes the terms under which the undersigned student (Participant) is authorized to participate in the University Games and related activities. Participation is conditional upon acceptance of the conditions, acknowledgements, releases, and consents set forth below. Acknowledgements, Assumption of Risk, Release and Indemnification

ASSUMPTION OF RISK: I acknowledge that participation in the University Games involves inherent risks, including but not limited to physical injury, illness, permanent disability, aggravation of existing conditions, and death. I voluntarily assume all such risks arising from my participation, whether known or unknown, foreseeable or unforeseeable.

RELEASE AND WAIVER OF LIABILITY: In consideration of being permitted to participate, I hereby release, waive, discharge and covenant not to sue the University, its trustees, officers, employees, volunteers, agents, event sponsors, coaches and representatives (Released Parties) from any and all liability, claims, demands, losses or damages on account of any injury, death, or property damage, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

INDEMNIFICATION: I agree to indemnify, defend and hold harmless the Released Parties from any and all claims, demands, liabilities, damages, losses and expenses (including reasonable attorneys' fees) arising out of or resulting from my negligence, willful misconduct, or breach of this Agreement.

I further understand that the Released Parties do not provide personal health or accident insurance for Participants and that I am responsible for obtaining my own insurance coverage.

I have read, understand, and voluntarily accept the Assumption of Risk, Release and Indemnification provisions above.

Consent for Emergency Medical Treatment

In the event of an injury or medical emergency, I authorize event staff, medical personnel, and representatives of the University to provide, secure, or arrange necessary medical care and transportation. I understand that reasonable efforts will be made to contact the emergency contact listed above prior to non-emergency treatments when feasible.

I consent to emergency medical treatment as described above.

Media Release

I grant the University and its authorized representatives the right to photograph, record, and use audio and video of me during the event in promotional, educational, and archival materials. I waive any right to inspect or approve such uses and release the University from any claims relating to the use of such media.

Transportation & Personal Vehicle Use

The University may provide or arrange transportation. If I drive personally to or during the event, I affirm that my vehicle is legally registered and insured and that I am authorized to operate the vehicle.

Standards of Conduct & Disciplinary Action

Participants must comply with all rules established by event organizers, team leadership, and University policies. Violations, including unsportsmanlike conduct, use of prohibited substances, hazing, or actions that endanger others, may result in removal from competition, suspension from future events, and referral to student conduct processes. The Participant consents to disciplinary measures and acknowledges that the University may take immediate action to protect safety and integrity of the event.
