What the Student Placement Document Covers

A Student Placement Document records the agreed terms for placing a student in an external program, internship, clinical rotation, or practicum. It identifies the student, educational institution, host organization, placement dates, responsibilities, supervision arrangements, insurance and liability allocations, and any specific learning objectives. This written agreement clarifies expectations for academic credit, attendance, background checks, health requirements, and data-sharing permissions so all parties understand obligations before the placement begins.

Why a Formal Placement Document Matters

A clear placement document reduces ambiguity about duties, protects student privacy, documents consent, and supports institutional compliance with FERPA, HIPAA when applicable, and applicable state education rules.

Why a Formal Placement Document Matters

Primary users and stakeholders

Each stakeholder must review and sign the document before the placement begins to ensure accountability and compliance.

  • Academic coordinators and internship offices who set academic requirements and approve host sites.
  • Host organization supervisors who confirm supervision, safety, and insurance coverage.
  • Students and parents/guardians who provide consent and agree to conduct and confidentiality terms.
