Medical Information & Consent

By checking the box below, the undersigned parent or guardian authorizes emergency medical treatment if the parent/guardian cannot be reached and acknowledges that reasonable efforts will be made to contact them.

I authorize emergency medical treatment and transportation as necessary for the student.

Athletic Code of Conduct & Policies

Players are required to adhere to standards of behavior that reflect positively on the athletic program and school. This includes attendance at practices, adherence to training rules, respect for coaches, teammates, opponents, officials, and compliance with all safety and equipment protocols. Repeated or serious violations may result in suspension or dismissal from the team.

The student and parent/guardian acknowledge receipt of the Code of Conduct, understand disciplinary procedures, and agree to comply with team rules and sanctions.

Concussion & Head Injury Policy

Concussion is a brain injury that must be properly evaluated. Any player suspected of sustaining a concussion will be removed from play and will not return until cleared in writing by a licensed health care professional trained in concussion management. Continued participation without clearance risks further injury.

The student and parent/guardian acknowledge the concussion policy and agree to immediate reporting and medical follow-up for suspected head injuries.

Substance Use & Testing

The athletic program maintains a drug- and alcohol-free environment. The school may implement testing protocols. Positive tests or refusal to submit to testing may result in immediate removal from competition and additional sanctions pursuant to policy.

The student and parent/guardian consent to the program's substance use policies and acknowledge potential consequences for violations.

Travel & Overnight Events

Participation in team travel is a privilege. Coaches will provide itineraries and supervision plans. Parents must supply emergency contact information and any medical details necessary for travel.

I grant permission for the student to travel with the team under school or coach supervision for scheduled events.

Equipment, Uniforms & Financial Responsibility

Players are responsible for care and return of issued equipment and uniforms. Families may be charged for damaged or unreturned items. Fees for participation must be paid in accordance with the program's schedule.

The student and parent/guardian accept responsibility for issued equipment and any applicable fees.

Academic Eligibility

Student-athletes must meet academic standards established by the school to remain eligible for participation. Coaches will monitor academic performance and may impose restrictions for failing grades.

The student and parent/guardian understand academic eligibility requirements and consent to release of pertinent academic records to coaching staff for eligibility verification.

Photo, Video & Media Release

The school may photograph or record students during athletic activities. Such media may be used for program promotion, publications, or educational purposes. Identifying information will be used in a manner consistent with school policy.

I consent to the use of non-confidential photographs and video of the student for program-related purposes.

Disciplinary Procedures & Grievance

Disciplinary measures are intended to be corrective and will be applied in a manner consistent with school policy. The student and parent/guardian are entitled to notice of allegations and an opportunity to respond prior to final sanction where feasible.

The student and parent/guardian acknowledge the disciplinary framework and grievance process set by the school and agree to participate in resolution procedures if needed.

Assumption of Risk, Release & Indemnity

Participation in athletics involves inherent risks, including but not limited to physical injury or death. The undersigned voluntarily assume such risks on behalf of the student. To the fullest extent permitted by law, the undersigned release the school district, its agents, employees, volunteers, and coaches from liability for ordinary negligence arising from participation and agree to indemnify and hold harmless the school and its representatives from claims, damages, or expenses resulting from the student's participation, except where such claims arise from willful or reckless misconduct.

The undersigned further agree that any dispute arising out of this handbook or the application of its provisions will be resolved through the school's established procedures and, where permissible, by other remedies available under law.

Acknowledgment & Certification

By signing below, the student (where of majority age) and the parent or legal guardian certify that they have read, understand, and accept the policies and requirements set forth in this Student Player Handbook. They certify that the medical information provided above is accurate to the best of their knowledge and consent to necessary emergency treatment. They agree to cooperate with coaches and school officials in enforcing handbook policies.

I acknowledge receipt of the Student Player Handbook and understand that it governs the conduct and participation of the student in the athletic program.

What the Student Player Handbook Is and Who It Covers

The Student Player Handbook is a formal, written set of policies, expectations, and administrative procedures governing student-athlete conduct, eligibility, safety, and administrative requirements for a team or athletic program. It centralizes code of conduct, medical and emergency procedures, travel rules, academic eligibility standards, equipment and uniform policies, and signature areas for consent and acknowledgement. The handbook is intended for use by the institution, coaching staff, student players, parents or guardians, and relevant administrative offices, and serves as the primary reference for program rules and obligations.

Why a Clear Handbook Matters for Teams and Programs

A well-structured Student Player Handbook reduces ambiguity about responsibilities, documents expectations for safety and eligibility, and documents consents and waivers that administrators and coaches rely on. Clear written rules help protect student welfare, support consistent enforcement, and create an auditable record for disputes or compliance reviews.

Why a Clear Handbook Matters for Teams and Programs

Primary Users and Stakeholders

The handbook is used by multiple parties who each have different responsibilities and access needs.

Each user group should know which sections they must complete, which records the program retains, and who to contact about questions or disputes.

Core Components to Include in a Professional Handbook

A complete Student Player Handbook should organize rules and forms into clear sections so signers can find and complete required items with minimal confusion.

Code of Conduct

Standards of behavior, disciplinary procedures, academic expectations, and consequences for violations; include reporting and appeals processes.

Medical & Emergency

Required medical disclosures, emergency contact details, concussion and injury protocols, medication policies, and HIPAA-related consent language where patient health information may be shared.

Consent & Waivers

Liability waivers, travel permission, and photo/media release forms with explicit signature and date fields for each signer.

Eligibility & Attendance

Academic eligibility rules, required documentation for enrollment, attendance expectations, and procedures for verifying status.

Travel & Housing

Travel itineraries, chaperone rules, overnight supervision standards, and any required parental or guardian authorization.

Equipment & Uniforms

Issuance and return policies, damage responsibility, replacement fees, and required maintenance.

Step-by-Step: Completing and Collecting Handbook Acknowledgements

A simple sequenced workflow helps ensure every required signer completes their sections and the program retains accurate records.

  • 01
Gather Documents: Collect handbook, consent forms, and medical release templates to be issued together.
  • 02
Pre-fill Administrative Fields: Program staff enters team name, season, and administrative dates to reduce signer errors.
  • 03
Distribute to Signers: Provide the handbook to players and guardians with clear deadlines and contact information for questions.
  • 04
Verify and File: Confirm required fields are complete, store signed copies, and record retention metadata.

Typical eSubmission Flow for a Handbook

Digital workflows follow a predictable sender-to-signer sequence and capture an audit trail for each action.

  • Upload: Program uploads handbook PDF and embeds signature, date, and checkbox fields.
  • Assign Signers: Add signer names and email addresses or generate a secure signing link.
  • Authentication: Signers authenticate by email link, SMS code, or stronger verification if required.
  • Completion & Archive: Signed records plus audit trail are delivered to all parties and archived in program records.

Configuring an Online Signing Workflow for the Handbook

Key settings ensure the digital handbook route meets institutional policies and preserves legal evidence of consent.

Field Configuration
Signing Order Sequential for guardian first, then student if required
Authentication Email link standard; use SMS or KBA for added assurance
Document Retention Store signed PDF with audit trail for required retention period
Notifications Automated reminders and completion notices to administrators

Technical Considerations for Digital Distribution

Choose a platform and settings that match your institution’s privacy, authentication, and retention requirements.

  • File Formats: PDF or DOCX accepted by most signing platforms
  • Authentication Options: Email, SMS, or multi-factor methods depending on sensitivity
  • Integration: Connectors to storage or SIS systems (e.g., Google Workspace, Microsoft 365) streamline archiving

Verify the platform supports audit trails, encrypted storage, and any compliance requirements such as HIPAA where student health information is included.

Typical Deadlines and Processing Expectations

Set and communicate clear deadlines for returning signed handbooks and required attachments to avoid eligibility delays.

Preseason Return Date:

All signed documents due before first practice or tryout

Medical Forms:

Submit updated medical releases and immunizations before travel

Roster Certification:

Coaches submit final rosters by institutional deadline

Late Submission Window:

Allow brief cure period but document exceptions in writing

Record Processing:

Administrators should confirm signatures and archive within 7 business days

Key Milestones from Distribution to Archiving

Track the handbook lifecycle as a sequence of milestones so stakeholders know where a form is at every stage.

01

Prepare Handbook

Finalize content and required attachments before distribution

02

Distribute to Signers

Issue documents with clear return-by dates and contact info

03

Collect Signatures

Monitor completions and send reminders for outstanding items

04

Verify and Archive

Confirm completeness and store electronic and/or paper copies securely

Common Mistakes to Avoid When Preparing the Handbook

  • Missing party names or mismatched legal names that prevent verification or cause eligibility disputes.
  • Unclear or conflicting policy language that leaves enforcement discretionary or inconsistent.
  • Failing to include HIPAA or FERPA statements where health or education records are shared.
  • Not tracking version history and retention metadata, which complicates audits and dispute resolution.

Consequences of Incomplete or Incorrect Handbook Records

Eligibility Delays: Incomplete forms can render a student ineligible until records are corrected
Regulatory Exposure: Improper handling of PHI or education records risks HIPAA or FERPA violations
Liability Claims: Missing waivers or unclear consents increase legal risk after injuries or incidents
Disciplinary Disputes: Poor documentation weakens positions in appeals or hearings
Financial Penalties: State or institutional fines may apply for recordkeeping failures
Data Breach Costs: Insecure storage can lead to remediation and notification expenses

Essential Data Elements for Compliance and Security

Student Identifier: Student ID or DOB for unique identification
Contact Details: Phone, email, and emergency contact
Medical Info: Allergies, medications, insurance
Consent Flags: Signed acknowledgement of policies and waivers
Version Metadata: Document version, issue date, and signer timestamps
Retention Tag: Retention period and disposal schedule
