Waiver, Release and Assumption of Risk

In consideration of the student’s participation in the activity described above, the undersigned, on behalf of the student and on behalf of the undersigned’s heirs, personal representatives, assigns and next of kin, hereby agrees as follows:

1. Release and Waiver — The undersigned releases and forever discharges Organization, its trustees, officers, employees, agents, volunteers, contractors and representatives (collectively "Released Parties") from any and all claims, demands, damages, liabilities, actions or causes of action, whether in tort, contract, or otherwise, arising out of or related to any loss, damage, illness, injury or death sustained by the student arising out of participation in the activity, whether caused by the negligence of the Released Parties or otherwise, except for willful misconduct.

2. Assumption of Risk — The undersigned understands and expressly assumes all risks associated with participation, including but not limited to transportation risks, weather-related hazards, physical exertion, contact with other participants, and risks inherent to the activity, whether foreseen or unforeseen.

3. Indemnification — The undersigned agrees to indemnify, defend and hold harmless the Released Parties from any loss, liability, damage, cost or expense (including attorneys’ fees) arising from the student’s participation or from any breach of this Agreement by the undersigned.

Media Release

I grant permission to Organization to photograph, videotape or otherwise record the student during the activity and to use such images or recordings for educational, promotional, or informational purposes in print or electronic media, without compensation.

I consent to the media release described above. I do not consent to the media release.

The undersigned certifies that the information provided on this form is true and complete to the best of their knowledge, that the student has permission to participate in the activity, and that the undersigned has read and understands the rules and policies governing the activity.
