In consideration for the student being permitted to participate in the identified activity, the undersigned, on behalf of the student, the parent(s) or legal guardian(s), and the student's heirs, assigns, personal representatives and next of kin, hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the school, its trustees, employees, volunteers, agents and representatives (collectively, the School Parties) from any and all liability, claims, demands, causes of action and damages arising out of or related to any injury, illness, death, loss or damage to person or property incurred in connection with participation in the activity, whether arising out of the negligence of the School Parties or otherwise, to the fullest extent permitted by law.

The undersigned accepts responsibility for any medical expenses incurred as a result of participation not covered by insurance and agrees to INDEMNIFY AND HOLD HARMLESS the School Parties from any claims, damages, losses or expenses arising from the student's participation.
