Duration, Limitation and Revocation

Revocation of this Agreement must be made in writing and delivered to the Institution's office responsible for student records and to the Representative. The Institution may rely on written or electronic notice of revocation once received by the Institution.

Privacy, Consent and Record Release

Student hereby authorizes the Institution and its authorized personnel to disclose and discuss Student's education and related records with Representative to the extent allowed by applicable student privacy laws and institutional policy. Student acknowledges that this consent is limited to the scope identified in this Agreement.

Student understands that certain records may be subject to additional restrictions and that the Institution may require additional forms or identification prior to release.

Representations, Liability and Indemnification

Student represents that the Student is the person identified herein and has the authority to grant the representation set forth. If the undersigned is a parent or guardian, the undersigned represents that he or she has the legal authority to act on behalf of Student.

Representative agrees to act within the scope of authority granted. Representative and Student agree to hold the Institution harmless from liability arising from the Institution's reasonable reliance on this Agreement. Student and Representative shall indemnify the Institution for claims arising from actions by the Representative outside the scope of authority granted by this Agreement.

Governing Law and Dispute Resolution

This Agreement shall be governed by and construed in accordance with the laws of the state in which the Institution is located. Any dispute arising under this Agreement shall be resolved by the forums available under institutional policy and applicable law.
