The Student agrees to perform assigned duties including set construction, rigging, lighting, sound, props, load‑in/load‑out, and other technical tasks as directed by the Crew Supervisor. The Student shall attend scheduled rehearsals, trainings, and safety briefings.

EQUIPMENT USE, PROPERTY, AND SAFETY

Student will use school equipment only as authorized and will follow manufacturer instructions, supervisor direction, and established lockout/tagout and maintenance protocols. Unauthorized use, modification, or removal of equipment is prohibited.

I accept responsibility for proper care and return of equipment while assigned to me, and I will report damage, loss, or malfunction immediately.

ATTENDANCE, TIMELINESS, AND DISCIPLINE

Consistent attendance and punctuality are essential. Repeated unexcused absences, failure to follow supervisor instructions, or conduct that jeopardizes safety or production may result in suspension from the crew or other disciplinary measures in accordance with school policy.

TRANSPORTATION

The Student/Guardian agrees to disclose medical conditions and to authorize emergency treatment when necessary. The Student/Guardian releases and agrees to indemnify the school to the extent permitted by law from claims arising from the Student's negligent acts or intentional misconduct while participating in crew activities.

TERMINATION

The school may suspend or terminate a Student's participation for cause, including but not limited to violations of safety rules, insubordination, repeated absenteeism, or conduct contrary to school policy. The Student/Guardian may withdraw the Student with written notice to the Crew Supervisor.

By signing below, the Student (or Parent/Guardian, if the Student is a minor) certifies that the information provided is true and complete, that they have read and understand this Agreement, and that they consent to the Student's participation subject to the terms set forth herein.
