The student and parent/guardian acknowledge and agree that maintaining a safe learning environment requires active compliance with school rules and reasonable standards of care. The student affirms the following commitments and understands the remedies for noncompliance described below.

Student will follow all safety rules and staff directives. This includes procedures for arrival, dismissal, emergency drills, and any temporary safety protocols implemented by the school.

Student will not possess or use weapons, illegal substances, or objects designed to harm. Possession or use will result in immediate disciplinary action and notification of law enforcement when required.

Student will promptly report safety hazards, threats, bullying, harassment, or concerning behavior to staff. Reports may be made anonymously when permitted; the school will investigate in accordance with policy and law.

TRANSPORTATION & OFF-CAMPUS ACTIVITIES

For the safety of all students, the undersigned grant or withhold the following permissions for routine school-sponsored travel and off-campus activities:

DISCIPLINE, REPORTING & REMEDIES

Noncompliance with this Contract or associated school safety policies may result in remedial measures, disciplinary sanctions, restriction of privileges, suspension from activities, restitution for damages, and notification of law enforcement or child protection authorities when required by law. The school will act in accordance with applicable mandatory reporting obligations for threats, abuse, or neglect.

EMERGENCY MEDICAL AUTHORIZATION

In the event of an emergency and the inability to contact the parent/guardian, I authorize school personnel to obtain or provide necessary medical treatment for the student. I understand that reasonable efforts will be made to contact the parent/guardian prior to non-life-saving interventions.

I consent to emergency medical treatment as described above and authorize the release of medical information to medical personnel, emergency responders, and school officials when necessary to secure care.

ACKNOWLEDGMENT
