I, the undersigned parent/legal guardian or student of legal age, certify that the student named above is physically and mentally able to participate in the snowshoe trip. I understand that snowshoeing and outdoor winter activities involve inherent risks including, but not limited to, uneven terrain, cold weather, hypothermia, slips and falls, and exposure to wildlife and natural hazards. I voluntarily assume all such risks on behalf of the student.

In consideration for permitting participation, I hereby release, waive, discharge, and covenant not to sue the school district, school, its officers, employees, volunteers, chaperones, and agents from liability for any and all claims, demands, actions or causes of action arising from personal injury, death, or property damage sustained during the trip, whether arising from the negligence of the school or otherwise, to the fullest extent permitted by law. I further agree to indemnify and hold harmless the school for any claims arising out of the student's conduct.

If medical treatment is necessary, I authorize school staff or designated chaperones to obtain and consent to any emergency medical care for the student, including transportation by emergency medical services and treatment by licensed medical personnel. I understand that efforts will be made to contact the parent/guardian or emergency contact before treatment when practicable. I accept responsibility for the cost of such care and transportation, including any deductibles under insurance coverage.
