Commitment and Conduct

By signing this Commitment, Student and Parent/Guardian acknowledge and agree that participation requires consistent attendance at practices and games. Student shall attend all scheduled practices and games except for illness, family emergency, or prior excused absence submitted in writing to the coach. Repeated unexcused absences may result in reduced playing time or removal from the roster.

Student shall adhere to the team's code of conduct, sportsmanship standards, and school or program academic eligibility requirements. Suspension or dismissal from the team may result from violations including but not limited to: persistent tardiness, unsportsmanlike conduct, use of illegal substances, academic ineligibility, or failure to comply with team or school policies.

Risk Acknowledgment and Release

Soccer is a contact sport that involves risk of injury, including serious injury. Student and Parent/Guardian understand these risks. To allow the Student to participate, Parent/Guardian hereby authorizes coaches and program representatives to obtain medical treatment for Student in the event of injury or illness when Parent/Guardian cannot be reached immediately.

Parent/Guardian acknowledges financial responsibility for medical care not covered by program insurance and releases the school or program, its employees, volunteers, and agents from liability for ordinary negligence. This release does not apply to gross negligence or willful misconduct.

I acknowledge the risk, authorize emergency medical treatment as needed, and accept financial responsibility for care not covered by insurance.

Transportation and Events

Student may travel with school-designated transportation to matches and events unless Parent/Guardian provides written notice granting alternate transportation. Parent/Guardian may permit Student to travel with another parent by submitting written authorization to the coach in advance.

Concussion Policy & Medical Return

Student and Parent/Guardian acknowledge receipt of the program's concussion information. If Student is suspected of having a concussion, Student will be removed from practice or competition and may not return until cleared in writing by a licensed medical provider in accordance with program and school policies.

Uniforms, Equipment & Financial Obligations

Parent/Guardian agrees to pay the program fee, uniform and equipment costs, and any fines assessed for loss or damage to issued equipment. Failure to pay outstanding fees may result in withholding of participation or withholding of finals/transcripts according to program policy.

Photo and Media Release

The program may record team activities and may use photographs, video, or audio of Student for promotional or educational purposes. Parent/Guardian may withhold permission by checking the opt-out box below; otherwise permission is granted for non-commercial use by the school or program.
