Assumption of Risk, Release and Indemnity

Assumption of Risk: I understand and acknowledge that participation in soccer involves inherent risks, including risk of serious bodily injury, permanent disability, paralysis and death, which may be caused by my child's actions, the actions of others, or the condition of the premises or equipment. I voluntarily assume all such risks associated with participation.

Release and Indemnity: In consideration of the student's acceptance into the program, I, on behalf of myself, the student, and our heirs, executors and assigns, hereby release, waive, discharge and covenant not to sue the organization, its officers, directors, employees, volunteers, coaches, agents and representatives (collectively "Released Parties") from any and all liability, claims, demands, causes of action, or losses for bodily injury, property damage or wrongful death arising out of or related to participation in the program, whether caused by the negligence of the Released Parties or otherwise. I also agree to indemnify and hold harmless the Released Parties from any loss, liability, damage or cost they may incur due to the participation of the student.

Severability: If any provision of this Release is held invalid, the remaining provisions shall remain in full force and effect.
