Medical Authorization: In the event of illness or injury during Spring Break activities, and if reasonable attempts to contact the undersigned are unsuccessful, I authorize Institution Name and its agents to obtain and consent to emergency medical treatment for the Student, including hospitalization, diagnostic procedures, anesthesia, and surgery as deemed necessary by licensed medical personnel. I agree to be financially responsible for such treatment and to promptly reimburse the Institution for emergency costs if paid by the Institution.

Conduct, Safety, and Prohibited Activities

Student agrees to comply with all applicable Institution policies, rules of conduct, and directions of chaperones or Institution representatives. Prohibited activities include, but are not limited to: illegal drug use, consumption of alcoholic beverages by minors, unauthorized removal from supervised activities, hazing, and any activity that endangers the health or safety of others. Violation of these provisions may result in immediate disciplinary action, including removal from activities at the student or guardian's expense.

Assumption of Risk; Release; Indemnification

Assumption of Risk: The Student and the undersigned parent/guardian understand that participation in travel and activities during Spring Break involves inherent risks, including travel hazards, recreational injuries, and unanticipated events. The Student and undersigned voluntarily assume all such risks.

Release and Indemnification: To the fullest extent permitted by law, the Student and undersigned parent/guardian release, waive, and discharge Institution Name, its trustees, officers, employees, agents, and volunteers from any and all liability, claims, demands, causes of action, or losses arising from or related to participation in Spring Break activities, including claims for negligence, except to the extent caused by the Institution's gross negligence or willful misconduct. The undersigned further agrees to indemnify and hold harmless Institution Name from and against all claims, damages, and expenses (including reasonable attorneys' fees) resulting from the Student's acts or omissions.
