STUDENT SUB AGREEMENT

Conduct: The student shall comply with all applicable school policies, the student code of conduct, and applicable federal and state confidentiality laws while performing duties. Failure to comply may result in immediate revocation of substitute privileges and disciplinary action by the School.

Confidentiality and Records

The student acknowledges that during the course of substitute duties the student may be exposed to confidential student records and personally identifiable information. The student agrees to maintain confidentiality in accordance with applicable law and School policies and shall not disclose such information except as required and authorized in the performance of assigned duties.

Eligibility, Background Check and Medical Information

Eligibility: The student affirms that he or she currently is a student in good standing and meets any age or enrollment requirements established by the School.

Liability, Indemnity and Status

Relationship: The student acknowledges that this Agreement does not create an employment relationship between the student and the School unless a separate written employment agreement is executed. The student performs substitute duties as a volunteer or under the School's student assignment policy as applicable.

Liability and Indemnity: To the extent permitted by law, the student (or parent/guardian if the student is a minor) agrees to indemnify and hold harmless the School and its officers, employees, and agents from claims, liabilities, losses, costs, and expenses (including reasonable attorneys' fees) arising out of acts or omissions by the student that are negligent or in violation of this Agreement.
