Subscription Term and Renewal: The subscription commences on the Start Date specified above and continues for the Term set forth in this Form. Unless Subscriber provides written notice of non‑renewal at least 30 days before the end of the then-current term, the subscription will automatically renew for successive terms on the same financial terms, unless otherwise amended in writing and signed by both parties.

Fees, Payment Authorization and Collection: Subscriber authorizes the Institution to charge the selected payment method for all fees, tuition, and applicable taxes. Late payments are subject to interest at the lesser of 1.5% per month or the maximum permitted by law, and Subscriber agrees to pay collection costs and reasonable attorneys’ fees incurred in enforcing payment obligations.

Cancellation and Refund Policy: Requests to cancel must be submitted in writing to the Institution. Refunds, if any, will be calculated in accordance with the Institution’s published refund schedule applicable at the time of cancellation. Early withdrawal does not relieve Subscriber of outstanding fees for services already rendered.

Conduct and Attendance: Subscriber and Student agree to comply with the Institution’s code of conduct and attendance requirements. The Institution reserves the right to suspend or terminate participation for violations that materially disrupt program delivery or present safety concerns.

Liability, Medical Consent, and Indemnification: Subscriber acknowledges that participation may carry risk. Subscriber authorizes routine medical treatment in the event of injury or illness and releases the Institution from liability for ordinary negligence to the maximum extent permitted by law. Subscriber will indemnify and hold the Institution harmless from claims arising from Subscriber’s or the Student’s misconduct or willful breach of this Form.

Data Use and Privacy: The Institution will collect and maintain Subscriber and Student information for administrative and educational purposes. By signing, Subscriber consents to the collection, use, and disclosure of personal information as necessary to provide the subscription services, including billing and emergency communications, in accordance with institutional policy.

Governing Law and Dispute Resolution: This Form will be governed by the laws of the state in which the Institution is located. Parties agree to attempt good faith resolution of disputes prior to commencing legal action. If legal action is necessary, the prevailing party is entitled to recover reasonable attorneys’ fees and costs.

Severability: If any provision of this Form is held unenforceable, the remainder of this Form will remain in full force and effect and the unenforceable provision will be replaced with a valid provision that most closely reflects the parties’ original intent.

Acknowledgment and Consent: By checking the box below and signing this Form, Subscriber certifies that the information provided is true and accurate, that Subscriber has the authority to enroll the Student (or is the Student if 18 or older), and that Subscriber accepts and agrees to be bound by all terms and conditions set forth in this Student Subscription Form.
