Attendance, Conduct, and Termination

The Student agrees to attend scheduled sessions punctually and to comply with program rules and staff directions. The Program Provider reserves the right to suspend or terminate participation for repeated unexcused absences, disruptive conduct, violation of safety rules or failure to follow reasonable staff directions. The Program Provider will provide written notice prior to termination where practicable.

Medical, Emergency, and Transportation Consent

Consent for medical treatment in emergencies and for transportation to and from program sites is authorized below if the parent/guardian cannot be reached and the situation requires immediate action. The Program Provider will exercise reasonable care in arranging and supervising transportation.

Liability and Indemnification

The Parent/Guardian or Student (if age 18 or older) understands that participation involves some risk. By signing this Agreement, the undersigned releases and agrees to indemnify and hold harmless the Program Provider, its employees, agents and volunteers from claims arising from routine participation, except to the extent caused by gross negligence or willful misconduct of the Provider.

Modification and Termination

This Agreement may be amended only by a written instrument signed by both parties. The Provider may suspend or terminate services for cause as described above; the Provider may also adjust scheduling or staffing due to enrollment, weather, or other operational requirements with advance notice when practicable.
