Scope of Authorization

For valuable consideration received, the undersigned hereby grants to the School/Institution named above, its affiliates, licensees and assigns, the irrevocable, perpetual, worldwide, royalty-free right to photograph, record, reproduce, edit, publish, distribute, exhibit, and otherwise use the Student's name, voice, likeness, image, performance, and biographical material (collectively "Materials") in connection with the production, promotion, distribution and educational activities of the School/Institution, in all media now known or hereafter devised, for any lawful purpose including but not limited to educational materials, promotional materials, social media, broadcast, web, and archival purposes.

This grant includes the right to create derivative works, to combine the Materials with other images, audio or text, and to edit or alter the Materials as the School/Institution determines. The undersigned waives any right to inspect or approve the finished product or any uses to which the Materials may be applied.

Release and Waiver

The undersigned releases and holds harmless the School/Institution, its officers, directors, employees, representatives and agents from any claims, demands, causes of action or liability arising out of or in connection with the use of the Materials, including but not limited to claims for invasion of privacy, defamation, or infringement of moral rights or personality rights, except to the extent such claims arise from the School/Institution's gross negligence or willful misconduct.

The undersigned hereby waives any right to royalties or other compensation arising from or related to the use of the Materials and agrees that the School/Institution shall own all rights, title and interest, including copyrights, in any works created using the Materials.

Duration; Assignment

This Release is perpetual, worldwide and irrevocable to the extent permitted by law and shall bind the undersigned and their heirs, legal representatives and assigns. The School/Institution may assign or transfer its rights under this Release without further consent of the undersigned.

Representation and Authority

The undersigned represents and warrants that they have the full right and authority to enter into this Release. If signing on behalf of a minor, the parent or legal guardian certifies they have the authority to grant the rights contained herein on behalf of the minor.

Privacy and Sensitive Information

The School/Institution will endeavor to avoid disclosure of sensitive personal information in conjunction with the Materials. The undersigned consents to the inclusion of the Student's name and program/grade with the Materials only if the undersigned selects the authorization below.
