The student agrees to attend all scheduled practices, meetings, and events unless excused in advance by the coach. Excessive unexcused absences or tardiness may result in suspension or removal from the team in accordance with team policy and school rules.

The student shall comply with the following standards while participating in team activities. Violation may result in disciplinary action up to and including dismissal from the team:

Demonstrate respect for coaches, officials, teammates, opponents, and school property.
Abstain from the use or possession of alcohol, illegal drugs, and performance-enhancing substances during team activities.
Follow directions of coaches and abide by written team rules and school discipline policies.
Maintain academic eligibility and promptly report scholastic issues to the coach.

Travel, Medical Authorization, and Emergency Care

The undersigned authorizes travel with team-appointed chaperones for scheduled competitions and related activities. The student and parent/guardian consent to emergency medical treatment deemed necessary by licensed medical personnel if the parent/guardian cannot be reached in a timely manner.

Discipline, Remedies, and Termination

Violations of this agreement or the team/school code of conduct may result in progressive discipline, which may include warning, suspension from competition, probation, restitution for damages, or removal from the team. The team reserves the right to terminate participation for cause where safety, repeated noncompliance, or serious misconduct is present.

Confidentiality and Use of Team Materials

Student and guardian agree not to disclose confidential team strategies, playbooks, or private personnel matters. Materials created by student as part of official team activities may be used by the team for educational and promotional purposes unless otherwise agreed in writing.
