Code of Conduct & Team Expectations

All team members are expected to conduct themselves with integrity, respect, and accountability. Unacceptable conduct includes, but is not limited to: intentional violation of safety rules, harassment, academic dishonesty affecting eligibility, misuse of team equipment, and repeated unexcused absences. Violations may result in progressive discipline up to suspension or dismissal from the team.

Team equipment and uniforms remain the property of the school/program. Members are responsible for care, timely return, and payment for loss or damage beyond normal wear.

I have read and understand the Code of Conduct and agree to abide by all team rules and policies.

Attendance, Practices & Competitions

Regular attendance at practices and competitions is essential. An absence is excused only for documented illness, family emergency, school-approved absence, or prior coach-approved reason. Repeated unexcused absences may affect playing time and standing on the team.

Academic Eligibility

Participation is contingent upon satisfactory academic standing as determined by school policy. Coaches will review academic status; failure to meet minimum GPA or other academic requirements may result in conditional participation or suspension.

I consent to periodic academic progress review by the coach or designated staff for eligibility monitoring.

Travel & Transportation

The team may travel to away competitions. Transportation will be provided according to program procedures. Parents/guardians must provide written permission for alternative transportation arrangements.

Permission for travel with the team:

Media, Photography & Social Media

The school/program may take photographs or videos of team activities for promotional and educational purposes. Student names may be used unless written objection is provided.

Media release consent:

Health, Safety & Concussion Protocol

All injuries must be reported immediately to coaching staff. Concussion symptoms will be managed in accordance with the program's concussion protocol; a medical clearance is required before return to participation following a suspected concussion.

I acknowledge I have received information regarding concussion signs and return-to-play procedures.

Discipline, Removal & Appeal

Discipline procedures are progressive and may include warnings, restricted participation, suspension, or removal from the team. Decisions by coaching staff may be appealed in writing to the program director within seven calendar days of notification; appeals will be resolved according to program policy.

Acknowledgment & Certification

By signing below I certify that the information provided is true and complete to the best of my knowledge, that I have read and understand the policies set forth in this Student Team Handbook, and that I accept the responsibilities and standards described herein. I authorize emergency medical treatment for my student if necessary and agree that the program, its employees, and volunteers are not liable for ordinary negligence related to participation.

I further understand that failure to comply with handbook policies may result in disciplinary action, including removal from team activities.

What the Student Team Handbook Is

A Student Team Handbook is a written guide that collects team governance, roles, responsibilities, conduct expectations, operational procedures, and signatory acknowledgements. It centralizes membership rules, emergency and medical protocols, and administrative processes so students, staff, and guardians understand obligations and the route for approvals, amendments, and dispute resolution.

Why a Handbook Matters and Its Legal Basis

A handbook creates consistent expectations, reduces disputes, and documents consent. For electronic execution, the handbook and individual acknowledgements can be enforceable under the ESIGN Act (15 U.S.C. ch. 96) and UETA (1999) where adopted, provided intent, consent, attribution, and retention requirements are met.

Why a Handbook Matters and Its Legal Basis

Who Relies on a Student Team Handbook

The handbook supports multiple stakeholders across school-based and extracurricular teams.

  • Student members, captains and officers responsible for following team rules and acknowledging conduct policies.
  • Parents or guardians when minors are involved, providing emergency contact and medical consent where required.
  • School administrators and coaches who approve policies, verify compliance, and retain signed records.

Use the handbook to align expectations among participants, parents, coaches, and school administrators.

Typical Signatories and Their Roles

Team Captain

Student leader who signs to acknowledge team rules, attendance expectations, and responsibility for peer conduct; acts as primary student contact for roster and equipment lists.

School Administrator

Designated staff member who authorizes the handbook, confirms alignment with district policy, and holds administrative authority to enforce sanctions or approve travel and budget items.

Core Sections to Include in a Professional Handbook

A complete handbook balances operational detail with clear signatory items. Include sections that address membership, conduct, safety, approvals, records, and amendment processes.

Team Overview

Purpose, mission, season schedule, and contact information for coaches, captains, and administrative liaisons.

Membership Rules

Eligibility criteria, attendance expectations, tryout and roster procedures, removal and reinstatement policies.

Code of Conduct

Behavior standards, academic expectations, disciplinary steps, anti-harassment protocols, and appeal process.

Medical & Safety

Emergency contacts, concussion and medical release authorizations, transport permissions, and required health clearances.

Travel & Finances

Trip approval workflow, travel supervision rules, fee schedules, refund and fundraising policies.

Amendment & Retention

How to propose changes, approval thresholds, version control, and record retention responsibilities.

Step-by-Step: Completing the Handbook

Follow a consistent sequence to prepare, review, and capture signatures for finalization and retention.

  • 01
Prepare Draft: Assemble policies, schedules, and required consent language for review.
  • 02
Review & Approve: Obtain administrative and legal review where district policy requires it.
  • 03
Collect Signatures: Issue acknowledgements to members and guardians and capture eSign or handwritten signatures.
  • 04
Store and Distribute: Archive the signed handbook and share copies with signers and relevant staff.

Configuring an Online Completion Workflow

Set up fields and routing logic to match approval steps and required signers when using an eSubmission process.

Field Configuration
Template Name Use a standardized file name including season and year.
Role-Based Signing Set signer order: member, guardian (if minor), advisor, administrator.
Notifications Enable email reminders and escalation after defined intervals.
Storage Destination Route completed files to secure cloud folder with restricted access.

Typical Submission and Filing Flow

A clear routing path reduces delays and ensures authoritative retention of the executed handbook.

  • Upload: Owner uploads final handbook PDF or DOCX to the signing platform.
  • Assign: Add signers and place signature, initial, and date fields.
  • Sign: Signers receive email or link, authenticate, and execute required signatures.
  • Archive: Signed copies and audit trail are stored in designated secure storage.

Technical and Integration Considerations

Ensure chosen tools meet district IT policy and data protection requirements before collecting sensitive student data.

  • File Formats: PDF and DOCX support for original and final copies.
  • Integrations: Connectors with Google Workspace, Microsoft 365, and Salesforce simplify distribution.
  • Authentication: Email verification, SMS codes, or stronger methods for sensitive approvals.

Common Timelines and Submission Deadlines

Define deadlines clearly to secure approvals, medical clearances, and roster confirmations before events or travel.

Annual Review Deadline:

At least 30 days before season start.

Roster Submission:

Submit final roster on or before first competition date.

Medical Clearances:

Due prior to first practice or per district timelines.

Parental Consent:

Collect before any supervised off-campus activity.

Policy Updates:

Distribute revised handbook within 14 days of approval.

Key Milestones From Draft to Active Policy

Milestones track progress from initial drafting through periodic review and archival.

01

Draft Publication

Policy language assembled and formatted for review.

02

Administrative Approval

School or district sign-off authorizes distribution.

03

Member Acknowledgement

Signatures collected from students and guardians as required.

04

Annual Review

Assess and update content for the next season or academic year.

Common Preparation Mistakes to Avoid

  • Inconsistent names across records causing identity mismatches and delayed approvals.
  • Missing guardian signatures for minors which can invalidate medical consent forms.
  • Unclear role descriptions that lead to conflicts and enforcement challenges.
  • Failing to version-control updates, causing multiple competing handbook copies in circulation.

Consequences of Incomplete or Incorrect Handbooks

FERPA Risk: Unauthorized disclosures can violate FERPA privacy rules.
Medical Liability: Missing releases may hinder emergency treatment decisions.
Discipline Disputes: Ambiguous policies increase appeal and grievance activity.
Contractual Gaps: Unexecuted acknowledgements weaken enforceability in disputes.
Recordkeeping Failures: Poor retention can impede audits and investigations.
Authentication Issues: Weak signer identity leaves signatures open to challenge.
