Acknowledgement of Policies, Release, and Assumption of Risk

By enrolling, the registrant and, if a minor, the undersigned parent or guardian hereby acknowledge and agree to the following terms: Tennis involves risks of physical injury, including serious injury and death. The undersigned voluntarily assumes all risks associated with participation, whether known or unknown, and releases, waives, and discharges the tennis program, its officers, employees, agents, contractors, and volunteers from any and all liability for injury, loss, or damage to person or property arising out of participation except to the extent caused by gross negligence or willful misconduct by the program.

The undersigned certifies that the student is physically able to participate in tennis activities, acknowledges the obligation to disclose any changes in medical condition, and agrees to comply with all program rules and staff instructions. Failure to comply may result in suspension or dismissal without refund if behavior endangers others or breaches program rules.

Cancellation & Refund Policy

All cancellations must be submitted in writing. Refunds for withdrawal prior to session start will be subject to administrative processing fees. No refunds for missed individual sessions unless cancelled by the program. The program reserves the right to cancel or reschedule sessions; in such event, registrants will receive prorated credit or refund as appropriate.

Disciplinary & Safety Rules

Participants must follow coach instructions, wear appropriate footwear, and adhere to facility rules. The program may remove participants for conduct endangering safety. Parents/guardians are responsible for timely pickup; repeated late pickup may result in additional charges.
