Assumption of Risk and Release: I acknowledge that participation in tennis and related athletic activities involves inherent risks, including risk of bodily injury. On behalf of the participant and myself, I knowingly and voluntarily assume all risks associated with participation and release and hold harmless the program organizers, coaches, employees, volunteers, facility owners and agents from any liability, claim, or cause of action arising out of injury, loss, or damage sustained in connection with participation, except to the extent caused by gross negligence or willful misconduct.

Indemnification: I agree to indemnify and defend the program and its representatives against any claims, costs, demands or suits arising from my child’s actions or omissions while participating, including legal fees incurred in defense.

Emergency Medical Authorization: In the event of an emergency, if I cannot be reached, I authorize program staff to obtain medical treatment for the participant and to arrange transportation to a medical facility. I understand that I am responsible for any medical expenses incurred.

Photo/Video Release: I grant permission for photographs and video recordings of the participant to be used for program promotional, educational, or archival purposes without compensation.   I do not grant permission.

Conduct and Dismissal: Participants are expected to comply with coach instructions and program rules. The program reserves the right to suspend or dismiss a participant for conduct that endangers others, or for repeated violations of program rules, without refund if dismissal is for disciplinary reasons.

Cancellation and Refund Policy: Registration fees are non-refundable after the session start date. Partial refunds for medical reasons may be considered upon receipt of written documentation; administrative fees may apply. The program reserves the right to cancel or reschedule sessions; in such event, participants will be offered a prorated credit or full refund at program discretion.

By signing below I certify that I am the parent/legal guardian of the above-named minor, or the participant if age 18 or older, that all information provided is accurate and complete, and that I have read, understand, and agree to the policies, releases, and authorizations set forth in this registration form.
