Attendance & Conduct: I acknowledge that regular attendance is required. Excessive absences or conduct disruptive to the class may result in dismissal without refund. I agree to comply with the Program Code of Conduct and directions of instructors and staff.

Photo/Video Release: I grant the program permission to photograph and/or record the student during class and performances and to use such images for educational, archival and promotional purposes. I understand no compensation will be provided for such use.

Medical Authorization: In the event of injury or illness, I authorize program staff to provide first aid and to seek emergency medical treatment as reasonably necessary. I authorize medical personnel to treat the student and to release medical information to program staff when needed for treatment and transportation.

Liability Release: To the fullest extent permitted by law, I release and hold harmless the program, its board members, employees, contractors and volunteers from any claims, liabilities, costs or damages arising from participation in program activities, except claims arising from gross negligence or willful misconduct. This release includes transport to and from off-site locations for rehearsals or performances.

Tuition & Refund Policy: The non-refundable registration fee secures enrollment. A full tuition refund (less registration fee) is available only if written notice is received at least 30 days prior to the session start date. After the session begins, refunds are prorated at the discretion of program administration; no refunds are provided for partial attendance beyond two weeks of classes. Unpaid balances may result in withholding of participation or performance privileges until paid.

COVID-19 and Communicable Disease Policy: I agree to notify program staff if the student has or is exposed to a communicable illness. The program may require temporary exclusion for illness, and may implement reasonable health measures to reduce risk of transmission.
