Liability, Indemnification and Data Use

By signing this Agreement, Parent/Guardian acknowledges that transportation is a privilege provided according to schedules and routes established by the Provider/School. The Provider/School shall exercise reasonable care in the provision of transportation services. Parent/Guardian releases and agrees to indemnify and hold harmless the Provider, the School, and their employees and agents from claims arising from ordinary risks of transportation, except to the extent caused by the Provider's gross negligence or willful misconduct.

The Provider and School may collect and retain personally identifiable information for legitimate transportation administration purposes, including routing, safety monitoring, and emergency response. Such information will be handled in accordance with applicable privacy and student records laws.

Termination, Changes and Notice

This Agreement may be modified or terminated by the Provider/School for operational reasons or for a material breach of this Agreement by Parent/Guardian or Student. Parent/Guardian may revoke or request modification of transportation services by delivering written notice to the School or Provider. Provider/School will make reasonable efforts to provide notice of route changes, but shall not be liable for disruptions caused by weather, mechanical failure, traffic conditions, public health directives, or other force majeure events.

Acknowledgment and Certifications

I certify that the information provided in this Agreement is true and complete to the best of my knowledge. I understand that falsification or failure to disclose material information may result in denial or revocation of transportation services. I authorize the Provider and School to take reasonable action in the event of an emergency, including seeking medical treatment for the Student when attempts to contact me are unsuccessful.
