The undersigned authorizes the school district and its authorized transportation provider to transport the named student to and from the designated stops consistent with the services requested above. Transportation services may be provided by district employees or contractors.

The undersigned authorizes school personnel and the transportation provider to take reasonable actions, including emergency medical treatment, when necessary to protect the health and safety of the student, and agrees to pay for any medical or other costs incurred as a result of such care. The undersigned grants permission for the transportation provider or school to release student contact and medical information to emergency responders when needed.

The undersigned acknowledges that video and audio monitoring may be used on vehicles for safety and disciplinary purposes and consents to such monitoring. The undersigned further acknowledges that failure of the student to comply with published bus rules may result in disciplinary action, suspension of transportation privileges, or other consequences as determined by school policy.

Except for acts of gross negligence or willful misconduct by the district or its agents, the undersigned releases and agrees to indemnify and hold harmless the district, its board, employees and contractors from claims, damages or liability arising from the provision of transportation services. Any alternate or temporary stop requests require written authorization from the school prior to the change being implemented.
