Assumption of Risk and Release: In consideration of the opportunity to participate in the trip identified above, I acknowledge that participation involves inherent risks including but not limited to transportation incidents, illness, injury, and property loss. I voluntarily assume all risks associated with participation to the fullest extent permitted by law. I agree to release, indemnify, defend and hold harmless the school, its board members, employees, volunteers, and agents from and against any and all claims, liabilities, losses, damages, costs, and expenses (including reasonable attorneys’ fees) arising out of or resulting from participation except to the extent caused by gross negligence or willful misconduct of the released parties.

Medical Authorization: If I am the parent or legal guardian, I authorize school personnel or designated trip leaders to obtain emergency medical treatment for my child if necessary. I understand I will be notified as soon as practicable. I agree to be responsible for any medical expenses not covered by the school’s insurance or my personal insurance.

Code of Conduct and Remedial Action: The student agrees to comply with all school policies, trip rules, instructions of trip leaders, and local laws. Failure to comply may result in immediate removal from the trip at the student’s expense, including transportation home and any additional costs. I understand that disciplinary action by the school may include temporary suspension from future activities.

Insurance: The school may maintain limited group coverage for the trip; parents/guardians are responsible for verifying their own health, accident, and travel insurance. The school is not responsible for personal property loss unless caused by its gross negligence.

Photo and Media Release: I hereby grant permission for photographs or video of the student during the trip to be used for educational or promotional purposes by the school, without further notice or compensation.

Cancellation and Refunds: Refunds for cancellations are subject to the written refund policy stated above in the Payment Plan or as otherwise set by the trip organizer. I acknowledge that non-refundable deposits may be required and that the student may be financially responsible for costs incurred on the student's behalf if the student withdraws or is removed.
