Liability Release & Emergency Treatment Authorization

In the event of a medical emergency, I authorize program staff to seek medical treatment for my child and to arrange transportation to an appropriate medical facility. I release the program and its employees and agents from liability for ordinary negligence arising from authorized medical treatment, to the extent permitted by applicable law. This release does not extend to willful misconduct or gross negligence.
