I, the undersigned, on behalf of myself or, if signing as parent or guardian, on behalf of the named student, certify that I have full authority to execute this Waiver and Release of Liability (the "Waiver"). I understand and acknowledge that participation in the activity described above involves inherent risks of injury, illness, or death. I voluntarily assume all risks associated with participation, whether known or unknown.

In consideration for the student being permitted to participate, I hereby release, waive, discharge, and covenant not to sue the school, its board members, officers, employees, volunteers, agents, and representatives (collectively, the "Released Parties") from any and all claims, demands, causes of action, liabilities, losses, or damages arising out of or connected with the student's participation, including but not limited to claims arising from the negligence of the Released Parties.

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, cost or expense (including reasonable attorneys' fees) incurred as a result of any claim, demand, or action brought by or on behalf of the student or any third party arising out of the student's participation.

I authorize school personnel to obtain emergency medical treatment for the student should the need arise and I am not reasonably available to grant authorization. I understand that reasonable efforts will be made to contact me prior to treatment when practicable. I agree to be financially responsible for the cost of such treatment.

I grant permission for the student's image, voice, or likeness to be photographed, recorded, and used by the school for legitimate educational, promotional, or archival purposes unless I indicate otherwise by checking the box below.

This Waiver shall be governed by the laws of the state in which the school is located. If any provision of this Waiver is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
